CHICAGO – September 2, 2025- The Academy of Osseointegration (AO) and the American Academy of Periodontology (AAP) have released a consensus report on the prevention and management of peri-implant diseases and conditions. The report is based on an evidence-based collaboration that brought together expert perspectives from periodontists, prosthodontists, general dentists, and oral surgeons actively engaged in implant therapy.
The joint effort, held in Oak Brook, IL, convened more than 40 participants who examined peri-implant health from both preventive and surgical standpoints. Divided into focused workgroups and plenary sessions, the groups developed practical, clinically oriented guidance to help clinicians better understand risk factors and therapeutic approaches associated with peri-implant diseases.
“We carefully outlined a stepwise approach beginning with non-surgical management of peri-implant mucositis and peri-implantitis, then progressed to surgical options such as osseous resective and regenerative procedures, as well as soft tissue augmentation,” said Hom-Lay Wang, DDS, MSD, PhD and a co-chair of the consensus workshop. “To guide clinicians, we created detailed flow diagrams offering clear pathways for both non-surgical and surgical care.”
Co-chair Paul S. Rosen, DMD, MS, emphasized the importance of risk awareness and prosthetic considerations. “We explored how prosthetic design, implant positioning, and soft tissue phenotype influence long-term implant health. Notably, we found strong consensus that tissue-level implants may help prevent peri-implant problems and identified factors such as restoration emergence angles and positioning that carry significant risk. Obesity was also highlighted as a potential risk indicator for peri-implant diseases.”

To further support clinicians, the AAP and AO will be producing a series of summary and in-depth discussion videos with the consensus authors and master clinicians. These resources will be released in the coming months on perio.org.
